Unless Pergola Crew Collective has entered into a written Business Associate Agreement with your organization covering the applicable Service:

  • Do not assume that the Service is approved for protected health information

  • Do not enter information that your organization is legally prohibited from submitting

A general website privacy policy does not replace a Business Associate Agreement or an organization’s own HIPAA Notice of Privacy Practices.
